Today, a patient visited us with dark knuckle pigmentation and lemon-yellow tinge of the eyes, which initially resembled mild jaundice. On closer examination and lab evaluation, we diagnosed the patient with severe Vitamin B12 deficiency.
The patient also had fatigue, glossitis, and borderline anemia with high MCV—classic signs of megaloblastic anemia. The lemon tinge was due to ineffective erythropoiesis, not liver dysfunction.
Vitamin B12 injections were promptly started, along with dietary advice. This case is a strong reminder that skin and eye changes may reflect serious nutritional deficiencies.
